  1. Remove signin, signout, and auth commands 

    Drop all GitHub/Microsoft authentication: signIn/signOut LSP calls,
device code flow, browser-based auth UI, enterprise URI config,
and the showDocument/showMessageRequest handlers. The plugin now
relies on g:copilot_api_url/g:copilot_api_key for API access.
