Approval leftovers: DM card in face rooms, pill with visible card, Always approve placement, timeline labels #271

Found in live re-verify on 275e957 (instance A):

• (a) A Bot's direct-message approval card (Helper → UiA1415's Chief) shows inside a Face-to-face room that merely contains the friend, with a "Waiting for the owner's approval" line, and both face rooms in the sidebar read "Waiting for your approval".
• (b) The top-right pill still shows while an approval card is already visible in the open chat.
• (c) "Always approve" sits left in some cards, right beside Reject in others.
• (d) Timeline annotation labels are small and sit on the lane edge; the red "waiting" text overlaps the lane border; the "waiting" label may not refresh right after approval.
