Every single edit is failing on the first attempt #1457
Description
I'm running in the VSCode terminal, relaunched using the command line to repeat the VSC built-in command (not sure if that's relevant, but it does mean I'm reusing the port number). I have not edited the file manually in between OpenCode's edits.

I experience this a lot with Claude sonnet, especially when there is 4-6 step work, after 1 or 2, it fails, with messages like "failed to execute the tool"
also the claude sonnet is so much slow on opencode, but its fast with their own cli / Cursor ( tested them all together )

I believe it was both Sonnet 4 and Opus 4, although I'm not 100% sure when the switch actually takes place if I switch mid-task.
