Only last file:// plugin loads when multiple plugins use index.ts entrypoints #15545

Description

When multiple local file:// plugins are configured and their entrypoints are all named index.ts, only one plugin ends up active at runtime.

In my real session, this caused Todo continuation behavior to be missing even though a todo was still in progress and the session became idle.

@fabriq/opencode_plugin_todo_continuation_enforcer, @fabriq/opencode_plugin_zellij, @fabriq/opencode_plugin_delete_session (all local file plugins resolving to .../index.ts)

Steps to reproduce

1. Configure multiple local plugins in opencode.json/opencode.jsonc (or merged config sources), where each resolves to a file://.../index.ts URL.
2. Start OpenCode.
3. Observe startup plugin logs and runtime behavior.
4. Only one of the configured local plugins is loaded/active.
