feat: add MCP resource subscription support

Add MCP resource subscription and template support so plugins can subscribe to resource changes and expose templated resources.

This issue doesn't fully meet our contributing guidelines.

What needs to be fixed:

No issue template was used. Feature requests must use the Feature Request template, which requires: a verification checkbox confirming the feature hasn't been suggested before, and a description of the enhancement.
The title should start with [FEATURE]: for feature requests.
The body is a single sentence with no meaningful detail — please describe the problem or need this feature addresses.
