Switch Session fetches a mixed session list and only filters root sessions in the dialog.
When a project has many newer child sessions, recent root sessions can disappear from the default dialog even though opencode session list still shows them.
Repro
Create a project with many recent child sessions / subagent sessions
Keep an older-but-still-recent root session
Open Switch Session
Notice that the root session can be missing from the default list
Why this happens
The dialog path limits the mixed session list before filtering parentID === undefined, so child sessions can crowd root sessions out of the first page.
No standard issue template was used. Bug reports must use the Bug Report template, which requires an environment/system information section (OS, OpenCode version, terminal, etc.) to help reproduce and diagnose the issue.

/sessions TUI only shows recent sessions, ignores historical ones #16270: Describes the exact same root cause (child session dilution in the TUI session list — mixed root+child sessions hitting the limit before client-side filtering). The author even references this issue. Please consider whether this adds new information beyond what is already tracked there.
Closing as a duplicate of #16270. That existing issue already tracks the same root cause: the TUI session dialog requests a mixed root+child list, hits the default limit, and only filters root sessions client-side, so recent child sessions can crowd root sessions out of the default picker.
