Skills aren't always refreshed when SKILL.md files are updated #23645
Description
Background
After editing a SKILL.md file and saving it, invoking the skill from opencode continues to use the previous (cached) content. Only restarting the opencode server causes the updated SKILL.md to be picked up. This makes iterating on skills slow and error-prone, as changes appear to be ignored until a full restart.

Acceptance Criteria
- Editing and saving a SKILL.md file is reflected immediately (or after a short file-watch delay) without restarting the server
- Cached skill content is invalidated when the underlying file changes on disk
