If you hover over the breakpoint, it should tell you why the breakpoint isn't bound. What does it say?
Are you using a launch.json file to start debugging, or are you launching through C# Dev Kit?
If launch.json, can you tell us what you your file looks like? If C# Dev Kit, which startup project is being launched? (example: App Host vs. front end, vs service)

There is no launch.json. I'm just hitting the F5 key (Start Debugging)

Enables the breakpoints again, but now the console is filled with Rpc errors:

Again, with the original launch setting, and the Release version of the C# and C# SDK extensions, breakpoints are working, and these errors don't appear in the console (or at least appear sporadically)
7. @jtsom-do I am assuming you have C# Dev Kit installed?
In your original configuration, the issue is a project system side problem where the debugger is being told to debug the AppHost process.
@gregg-miskelly Yes, I have the C# Dev Kit extension (Where I said "C# SDK" that's what I meant. My bad)
In other words, the pre-release versions of the C# and C# Dev kit are only debugging the AppHost, and ignoring the other projects in the app? The release versions can debug all projects in the app.

The problem is only with the pre-release version of C# and C# SDK (v11). Switching to the release versions of the extensions fixes the problem.
This only does seem to happen with an Aspire based app. Other multi-project solutions do not show this problem.
