Describe the bug
In a particular session, at a certain point, none of my prompts are getting executed, just getting queued. In the CLI I can see MCPs getting reconnected ad nauseum even though they are already connected. Exit and restart CLI, resume session did not help, hit the same issue. I had to abandon the session and start afresh.

Steps to reproduce the behavior
No particular steps to repro, repros randomly in certain sessions. I have hit this 2 times in recent memory.
Expected behavior
Prompts get executed, session makes progress.
