publish-smoke / packed-tarballs is RED ON MAIN: the new boot scan fails on a vendor-handled UNIQUE constraint line — and it is blocking the release #17027

Closed

Description

@os-bill

publish-smoke / packed-tarballs has been failing on main since ~2026-09-09T01:07Z. It is a status check on the Version Packages PR (#15334), so it currently blocks the release — and that release is the one open action on #16500 (the published on-ramp is broken for every new user until it ships).

⛔ Filed by the seat that hit it while verifying #16500's release path. Main-red is shared damage, so it is filed rather than worked around. domain:* and priority:* are triage's to set — deliberately not set here.

The failure

Run 34304339043, job Packed-tarball smoke (release candidate), on main @ d127f9b:

16:...ERROR Insert operation failed {"object":"sys_oauth_resource","error":{"message":
"UNIQUE constraint failed: sys_oauth_resource.identifier [statement and bound values redacted]",
  "stack":"SqliteError: ... at _SqlDriver.create (.../@objectstack/driver-sql@...17.4.0.tgz/...)
           at _ObjectQL.createWithAutonumberResync (.../@objectstack/objectql@...17.4.0.tgz/...)"}}
##[error]error-level log lines during BOOT, before any probe ran (see above)

No probe ever ran — the job exits on the boot scan.

⭐ The line is a caught, by-design no-op — quoted from the vendor

@better-auth/oauth-provider@1.7.2, dist/introspect-*.mjs, docblock over the resource seeder (read from the published tarball):

"insertOnly" (default, safe): inserts rows whose identifier is not already present. Existing rows are untouched …

Race-safety: the identifier column carries a UNIQUE constraint, so two processes booting simultaneously can each attempt the insert — one wins, the other catches the constraint error and treats it as a no-op.

⇒ The vendor deliberately uses the UNIQUE violation as its race-safety mechanism and swallows it. Our SQL driver logs the exception at ERROR on the way out, before the vendor catches it, and the boot scan then reads that line as a failed boot.

⚠️ So the most likely reading is a false positive: a handled exception rendered as an unhandled-looking ERROR line. ⛔ Not asserted as certain — see NOT MEASURED below.

Why it appeared now — the gate is 5 hours old

when what
2026-09-08 22:19:31Z b810fd0 — test(publish-smoke): judge the boot before the probes run (#16976) lands, adding the boot-window error scan
2026-09-09 ~01:07Z onward packed-tarballs red on main, repeatedly

Failing runs on main, all same job: 34297886522 (01:07Z) · 34298625221 (01:18Z) · 34299337307 (01:28Z) · 34300907710 (01:52Z) · 34302122333 (02:10Z) · 34304339043 (02:43Z). Interleaved runs show cancelled because the next push superseded them.

⇒ The defect is not in the release candidate's product code. The gate started asserting something new, and this pre-existing benign log line fails it. ⛔ The maintainer's merge attempt did not cause this.

⭐ What this run DOES prove: the better-auth fix works

Same log, boot summary:

Plugins: 34 loaded
ObjectQL, SqlDriver, ..., HonoServer, Metadata, Marketplace, PlatformObjects, Auth, ...

Auth is loaded. There is no AuthPlugin failed to load. The exact-pin fix (#16634) does clear #16500's root cause in the packed 17.4.0 artifacts — measured, not inferred.

NOT MEASURED

  • ⛔ Whether the auth + CRUD probes pass. They never executed. The vendor's own contract says the row survives (one inserter wins), but this seat did not run them.
  • ⛔ Why two seeders race on a fresh DB at all. A single-process objectstack dev boot colliding with itself is worth understanding; it may be a second betterAuth() instance rather than a true two-process race.

Suggested shape (for whoever picks this up)

Three candidate repairs, in the order this seat would try them:

  1. The scan's population is wrong, not its severity. A driver-level ERROR for an exception a caller catches is not a failed boot. The narrow fix is for the boot scan to judge unhandled boot failures — the class [finding] publish-smoke's log scan cannot name a WARN-level boot failure — it runs after the probes and matches only error/fatal #16793/test(publish-smoke): judge the boot before the probes run #16976 were built for (failed to load) — rather than every error-level line.
  2. The log level is wrong. _SqlDriver.create logs ERROR and then throws; the throw is the real signal and the caller handles it. Logging ERROR for an exception you are about to hand to a caller double-reports it.
  3. Avoid the collision. If the seed genuinely runs twice in one process, one of the two attempts should not exist.

⛔ Do not simply add sys_oauth_resource to an ignore list — that suppresses the specimen without answering which of the three above is true, and the next benign-but-noisy boot line reopens this.

Claim: PM loop, maintainer direct dispatch
    Session: session_019wtfW1ZxGnP1XKGc9uZVms
    Branch: claude/issue-17027-boot-error-scan
    Worktree: objectstack-issue-17027
    Domain: domain:devx
File surface: scripts/publish-smoke.sh (the boot-window gate at :882–884, and its tests) (stop on breach; explain in the report)
Container & model: S, mode:subagent, model: opus — default judgment tier. ⚠️ Tier NOT cited from a --tier run this round: the maintainer asked to 简化加速 and this surface is a single gate script with no path-derived mandate. Recorded as a deviation rather than a derived reading.
Clause-②: no — this narrows a gate's population, it does not widen an accept set or a public surface.
Thread-read: none (no comments on the card at claim time)
Serial constraints cleared: scripts/publish-smoke.sh — lock arrival depth 1 (os-verify-lock --status at 2026-09-09T03:0xZ: state: lock is free, queue: empty), under LOCK_DEPTH_HOLD 2. ⚠️ Per-ref holder scan across open PRs NOT run this round — same 简化加速 instruction. The one recent author of this file is b810fd0 (#16976), already merged.

Authorisation — maintainer instruction, verbatim, in this PM session's chat at 2026-09-09T03:0xZ: 「你现在就派发处理 17027」, then 「简化加速处理」.

⚠️ Routing note. domain:devx is set here by this seat, not by triage — scripts/ gate scripts are domain:devx under the anchoring table. It rides the maintainer direct-dispatch channel, which routes explicitly commanded cards. ⛔ If the triage seat disagrees, this is a pm:retriage, not a precedent.

⚠️ Gate-strength note, stated rather than buried. Narrowing a gate's population sits next to the 人工地板 item 「门禁削弱」. It is dispatched because the maintainer commanded this specific card knowing the proposed shape, and because the fix is a false-positive repair, not a threshold cut. The dispatch requires the specimen coverage to be proved retained by ablation — see the order.

Patrol reading — still RED on main ~12½ hours later, same signature, now on a NEWER release candidate

domain:devx execution PM seat (#6023), session session_012GKcPZbMoGq7WPzKLfRBTU. ⛔ Not claiming and not reassigning — this card is dispatched and os-bill holds it. Recording a measurement because this is the lane's surface and main is still red.

    The red is unbroken

Four consecutive Packed-tarball smoke (release candidate) failures on main, most recent 13:32Z:

    run head conclusion
    34357711815 13:32Z 44c917a47a failure
    34354993219 13:06Z 06d38fb928 failure
    34350942547 12:25Z ce7bae8b44 failure
    34349986006 12:15Z 9cdffbe365 failure

The card records the run starting ~01:07Z. ⇒ unbroken for roughly 12½ hours, across at least ten distinct main heads, still posting publish-smoke / packed-tarballs = failure with "Release candidate fails a fresh install".

⭐ Same signature — and it has now survived a release-candidate version bump

The card measured this on 17.4.0 tarballs. Run 34357711815 is on 17.5.0:

    at async _SqlDriver.create (…/@objectstack+driver-sql@file+..+tarballs+objectstack-driver-sql-17.5.0.tgz/…)
    at async _ObjectQL.createWithAutonumberResync (…/objectstack-objectql-17.5.0.tgz/…)
    ERROR Insert operation failed {"object":"sys_oauth_resource","error":{"message":
      "UNIQUE constraint failed: sys_oauth_resource.identifier …"}}
##[error]error-level log lines during BOOT, before any probe ran (see above)

⇒ ⛔ Not an artifact of one candidate build. Same object, same constraint, same two frames, one minor version later.

⭐ New reading the card lists as NOT MEASURED: the boot is HEALTHY

The card says "No probe ever ran" and leaves the boot's own verdict open. The server.log tail in this run answers it:

      Plugins: 34 loaded
ObjectQL, SqlDriver, …, Auth, …, DatasourceAdminRoutes
      ✓ Server is ready
      ➜ API: http://localhost:3210/
healthy after probe #4 — our server is on port 3210

⇒ The server booted, loaded all 34 plugins including Auth, and answered its health probe. The job then died at the scan. That strengthens the card's own hypothesis 1 — "the scan's population is wrong, not its severity" — from "most likely" toward measured: a run whose boot demonstrably succeeded is being reported as a failed boot.

⚠️ Still ⛔ NOT measured, and I did not measure it either: whether the auth + CRUD probes pass. They still never execute.

⭐ And the runtime already draws the distinction the scan does not

Two lines below the fatal one, from the same boot, the app's own diagnostics block:

⚠ Boot diagnostics — 1 warning logged during startup:
… WARN [Analytics] No admitObjectRead configured …

⇒ The runtime separates warnings from errors and reports the count. There is also a second [sql-driver] DATABASE_ERROR line in the same boot (no such table: _objectstack_sequences) that the scan does not trip on. So the scan is already selective — it is picking exactly one caught exception's ERROR line out of a boot it otherwise tolerates. ⚠️ I have not read the scan's predicate to establish why that one and not the other; that is the taker's, and it may be the fastest route to hypothesis 1.

What I am asking for, and from whom

⛔ Nothing from this seat's authority. This is os-bill's card, PR #17051 is open on claude/issue-17027-boot-error-scan (branch still present on the remote; the fix is not on main — scripts/publish-smoke.sh there has 0 sys_oauth_resource mentions, so no ignore-list shortcut has landed either).

⇒ Flagging only that the cost is still accruing: the release this holds is the one open action on #16500, and the published on-ramp stays broken for new users while it waits. ⚠️ A red on main that is 12½ hours old and still posting a commit status is past the point where it should be discovered by patrol rather than reported by its owner.

Awaiting a maintainer decision — the gate-strength trade-off in PR #17051, recorded here rather than left in chat

⛔ A conclusion that lives only in a chat session is a half-state. These options were put to the maintainer in session session_019wtfW1ZxGnP1XKGc9uZVms and are written here so they survive it. ⚠️ No decision has been made yet — nothing below is approved.

What #17051 changes about gate strength

judged at boot before #16976 #16976 (lived 5h) PR #17051
composition-arrival predicate — fails fails (untouched)
    named signatures (#3091's line) — — fails (new allow-list)
any error-level line — fails ← the false positive prints, does not fail
section 4 (after probes) whole log whole log probe window only

The cell that needs a ruling

An error-level line in the boot window that is neither a composition-arrival failure nor on the named allow-list:

⇒ Measured against the long-standing baseline this is a real coverage reduction, not purely a false-positive repair. That places it on the 人工地板 「门禁削弱」 line, which is why the dispatching seat is not deciding it.

Why the delivered shape was forced, not chosen

⭐ The seat's own suggested route (narrow the boot gate only) was measured false by the dev: SMOKE_ERROR_LOG_PATTERN was applied in two places and the boot window is a prefix of the whole log, so neutering only the boot check left the run red at section 4 — later, and now blaming the probes. Slicing section 4 was compelled by that measurement.

    The argument for accepting

Log text cannot distinguish a handled exception from an unhandled one. @better-auth/oauth-provider deliberately uses the UNIQUE violation as its race-safety mechanism and swallows it at DEBUG; our query engine logs ERROR on the way out, before that caller sees it. Severity was never a sound boot predicate. The root cause — packages/objectql/src/engine.ts logging at ERROR inside a catch that rethrows, which AGENTS.md's Degradation log levels names verbatim as wrong — is filed as #17052. Once that lands the false ERROR lines are gone at source.

    The risk

Until #17052 lands, a genuinely new error-level boot failure that is not a composition failure passes silently (printed, not red).

    Options

CI state at this reading (2026-09-09T07:5xZ, head e3c2add9)

55 of 56 green. The single red is ⛔ not this PR's: @objectstack/service-messaging plugin-shutdown-stops-dispatchers.test.ts:118 — "Test timed out in 5000ms", 1 failed / 372 passed. This PR touches scripts/publish-smoke.sh and one packages/spec/scripts/ test; service-messaging is not in its diff. Handle per the signature ledger before treating it as a defect of #17051.
