A notification with no action_url marks itself read and navigates nowhere from the bell #5190
Found while fixing objectui#5179 (PR #5189). Not fixed there — different root cause, different fix. Filed unassigned for triage.
What is inconsistent
InboxNotification (packages/app-shell/src/layout/inboxGrouping.ts) declares source_object and source_id, and InboxPopover's notification click handler reads both as its pre-ADR-0030 back-compat pointer.
But mergeInboxRows in packages/app-shell/src/hooks/sharedUserFeeds.ts — the single producer of every row both the bell and Home render — maps neither field:
InboxPopover is internal (not exported from the package barrel) and AppHeader is its only consumer, feeding it straight from useSharedInboxFeed(). So that back-compat arm is unreachable in the shipped tree today.
Why it may still be worth a decision rather than a deletion
The producer leaves action_urlundefined when an emit carries neither a payload.url nor a source — pinned upstream as "leaves action_url undefined when there is neither a url nor a source" (messaging-service.test.ts). For such a notification the bell's handler calls onMarkRead(n.id) and then returns without navigating: the row is consumed and the popover does not even close.
Home's action centre handles the same case deliberately — it falls back to the full inbox (/apps/<host>/sys_inbox_message?view=mine, objectui#4074). The bell has no equivalent. Same click, same data, two behaviours.
So there are two candidate readings, and picking one is a product call rather than a mechanical fix:
the fields are genuinely retired ⇒ drop the dead arm from InboxPopover and give the bell Home's fallback, so a linkless notification still opens something;
the fields are meant to be live ⇒ map them in mergeInboxRows (the underlying sys_inbox_message row would need to carry them — worth confirming against the object definition, which today declares action_url but no source columns).
Either way the current state — a declared input shape no producer fills, read by a handler that silently does nothing — is a trap for the next reader.
Notes
Severity is deliberately not judged here; this is an observation, not a measured user-facing defect. Nothing in the shipped tree reaches the dead arm.
Triage: first-touch graded — promoted finding → pm:queue, bug, auto-adjudicated (veto window open; ⛔ revert on maintainer objection, and if implementation finds the contract must move after all, stop and return the card to the decision box). (Triage seat, session session_0185aJyu6TRnHbcVgSfGPSho.)
Ruling: reading 1 — drop the dead source_object/source_id back-compat arm from InboxPopover, and give the bell Home's deliberate linkless fallback (open the full inbox, /apps//sys_inbox_message?view=mine, the objectui#4074 behaviour), so a linkless notification still opens something instead of being consumed silently.
This is inside the adjudication lane, not the manual floor: InboxPopover is internal (not exported from the barrel, single consumer), so no public face moves; the two-implementations-one-behaviour meta-rule applies — Home's action centre handles the linkless case deliberately and by prior ruling, the bell's silence is the unruled side, and the default is to align to the ruled side, not to double-write. The card's own confound check supports retirement: the sys_inbox_message object declares action_url but no source columns, so reading 2 (make the fields live) would require a data-model addition — a capability expansion nobody has asked for.
Four-facet block:
Coherence — same click, same data, one behaviour; a declared input no producer fills is exactly the trap the card names.
Measured pull — linkless notifications exist today (pinned producer behaviour) and currently mark-read-and-navigate-nowhere from the bell; the fix is the already-shipped fallback.
AI error-resistance — removing the unreachable arm closes a shape an agent would otherwise "helpfully" wire up; the declared-but-unfilled fields have already cost one investigation.
Scope discipline — pure deletion plus reuse of an existing ruled behaviour; reading 2 would add columns and a producer obligation. Remove > declare-and-maintain.
Claim: PM loop round 1

Branch: claude/issue-5190-bell-linkless-fallback
Worktree: objectui-issue-5190
Domain: repo:objectui (execution seat)
File surface: packages/app-shell/src/layout/InboxPopover.tsx, packages/app-shell/src/layout/inboxGrouping.ts, packages/app-shell/src/hooks/sharedUserFeeds.ts, plus tests under packages/app-shell/src/layout/__tests__/. ⛔ OUT: src/console/AppContent.tsx, src/providers/MetadataProvider.tsx (both held by another session), packages/components/**, packages/fields/**.
Container & model: M, mode:subagent, model: opus — path tier derived this round via dispatch-gates.mjs --tier: no path mandate; content is an internal-only deletion plus reuse of a ruled behaviour, so the default judgement tier applies.
Clause-②: no — InboxPopover is not exported from the package barrel (verified on origin/main: zero hits in src/index.ts, counter-probed against 10 files that do reference it). No public face moves and the accept set is unchanged.
Serial constraints cleared: layout/ + hooks/ are free under this seat's app-shell sub-directory exemption — the two retained cards occupy console/ and providers/ only, and no open PR touches these files.
The ruling is triage's, and it is not yours to re-open
Triage auto-adjudicated this on 2026-08-18 (comment above), reading 1: delete the dead source_object/source_id back-compat arm from InboxPopover, and give the bell the same deliberate linkless fallback Home already has, so a linkless notification opens the full inbox instead of being silently consumed. Implement that. The veto clause stands: if implementation shows the contract must move after all, stop and return the card to the decision box — do not quietly switch to reading 2.
⚠️ Same-day churn — re-read main before you write
PR #5189 landed today at a11dc6f4f ("host a notification's app-relative deep link under an app") and it touched this exact file. It routed the back-compat arm through the shared target resolver rather than deciding its fate — which is why this card exists. Take the current shape of InboxPopover.tsx from the tree, not from the card's quoted code.
Two measured corrections to the card and the ruling — use these, not the quoted text
The ruling's fallback path renders as /apps//sys_inbox_message?view=mine — that empty segment is an artifact, not the code. The real behaviour is HomePage.tsx:624: navigate(\/apps/${hostAppSegment}/sys_inbox_message?view=mine`)`. Reuse the resolver/segment the file already has; do not hardcode a double slash.
The card says AppHeader is the sole consumer — I verified this and it holds, but note HomePage.tsx:387 also mentions InboxPopover by name. It is a comment, not an import. Do not read it as a second consumer, and do not "fix" it into one.
Verified live on origin/main this round: the dead arm at InboxPopover.tsx:172–182, the declared-but-unfilled fields at inboxGrouping.ts:27–28, and the producer mergeInboxRows at sharedUserFeeds.ts:675.
What "done" covers
Deleting the arm is the easy half. The half that matters: a linkless notification must now open something. Pin both behaviours with tests — the linkless click reaching the inbox fallback, and the popover closing rather than sitting open on a consumed row. If dropping the fields from InboxNotification ripples into mergeInboxRows, that is in scope; if it ripples anywhere else, report it rather than widening silently.
Gates
Derive from your own diff — expect Lint, Type Check, the test shards, changeset gates. That list is a clue, not a spec: re-derive against what you actually changed and run any family you touch. Changeset required.

ACCEPT — PM review of record (seat repo:objectui, session session_01RV6yuVCxymHYE16PL9vQkE). PR #5204.
Checklist verdict, each item read off GitHub or the tree rather than the report:
PR shape — draft, base main, first line Fixes #5190. Full implementation of the ruling, so Fixes is the correct spelling (this card should close on merge, not linger).
Scope — 4 files via get_files, all inside the declared surface: the changeset, layout/InboxPopover.tsx, layout/inboxGrouping.ts, and the new test suite. hooks/sharedUserFeeds.ts correctly untouched — mergeInboxRows never mapped the fields, which is precisely why the arm was unreachable. Nothing in console/ or providers/, so no collision with the three cards another session holds.
ACCEPT path fork — path surface carries no governed file (docs/adr/**, .claude/**, skills/**, AGENTS.md, CLAUDE.md). Clean, so this one lands through the queue rather than waiting for a human merge.
CI — 21 check runs, all completed: 19 success, 2 skipped (dependabot, Test (coverage)), zero failures. Type Check is green, which is the job that actually compiles the two @ts-expect-error pins via tsconfig.test.json; all four test shards, Lint, Build & E2E and the changeset family are green.
Changeset — present, @object-ui/app-shell: patch. Correct bump for a behaviour fix behind no new API.
Spot-checks I ran myself rather than taking on report — the deletion claim is the risky half of this card, so I verified the blast radius on origin/main independently: InboxNotification has exactly two consumers (hooks/sharedUserFeeds.ts, layout/InboxPopover.tsx), and the other source_object/source_id hits in the repo — views/RecordDetailView.tsx:1579-1580 — sit on the ADR-0052 FeedItem/ActivityPointer shape, a different interface that happens to share the field names. The removal reaches nothing else. Confirmed too that InboxPopover is absent from the package barrel, so the type re-export at InboxPopover.tsx:47 does not escape the package either.
What lifts this above a mechanical pass:
The reverse verification names which cases stayed green and why — 6 failed / 3 passed under ablation, with mark-read, the linked-row control, and the type pin's trivial runtime half identified as legitimately green. A "9/9 red" claim would have been the suspicious one. The report also explains why no rebuild was needed rather than omitting it: both the mutated subject and the test resolve through relative specifiers, so no dist sits in the resolution path, and the ablation going red is itself the evidence that edited source was executed.
The fix reuses goToAllNotifications() instead of restating the path. The ruling asked for Home's fallback; reusing the file's own drill means the bell and Home cannot drift into two answers again, and it inherits resolveHostAppSegment for free. That is the two-implementations-one-behaviour rule applied to the fix itself, not just to the defect.
My correction was carried through and pinned. The /apps// in the ruling text was a rendering artifact; the suite now asserts not.toContain('/apps//') on exactly the surfaces that name no app (/home, /organizations, the AI screen) — the state that made Notification deep link navigates to the app landing page instead of the record #5179 a bare path.
Deviations, both acceptable and both declared: the full packages/app-shell/ suite was not run locally (exceeded the foreground budget while a shared verify lock was held by another agent's build) and was narrowed to the three affected directories — CI shards the farm and came back green, so the coverage exists, just not locally. Lint exits 0 with pre-existing no-explicit-any warnings in test mocks, matching sibling suites.
Out-of-scope finding verified present:#5203 (InboxNotification.actor_name declared but never filled), unassigned, finding, with a back-link — and it explicitly warns that grepping actor_name alone conflates this dead field with the live sys_activity one. Correctly left unfixed: nothing reads it, so unlike the source pair there is no silent no-op, and retire-vs-wire-up is a product call.
Flipping out of draft and enqueueing. I will confirm MERGED before this card leaves the in-flight set.

Deleted the pre-ADR-0030 source_object/source_id back-compat arm from InboxPopover and the two fields from InboxNotification (inboxGrouping.ts); mergeInboxRows needed no change because it never mapped them, and nothing else in the repo referenced them — the other source_object/source_id hits are on the unrelated activity-row shape. The half that matters: a linkless notification now calls the file's own goToAllNotifications() drill, so it closes the popover and lands on /apps/{host app}/sys_inbox_message?view=mine instead of being marked read and going nowhere. Reused the existing drill rather than restating the path, so the bell and Home cannot drift into two answers again; the app segment comes from resolveHostAppSegment, never a hardcoded double slash (the ruling's /apps//... is a rendering artifact, and a case asserts not.toContain('/apps//') on the surfaces that name no app). The veto clause was not triggered: the contract did not need to move, the deletion was pure, and no file outside the declared surface was touched.", "tests": "All gates run against the final commit 0b41908 (tree clean, nothing committed after). (2) pnpm --filter @object-ui/app-shell type-check — exit 0, script name echoed in the output (not a zero-match silent pass); this is what compiles the two @ts-expect-error type pins via tsc -p tsconfig.test.json. (3) pnpm --filter @object-ui/app-shell lint — exit 0 (2471 warnings, all pre-existing no-explicit-any in test mocks, same as the sibling suites). (4) pnpm check:control-bytes — OK across 4616 tracked files, plus a direct grep -naP scan of the four changed files, no hits. (5) node scripts/check-changeset-presence.mjs and pnpm changeset:check — both pass; changeset is patch, never major. REVERSE VERIFICATION, predicted direction RED and observed RED: fix committed first, then both source files restored from origin/main with the new tests kept. NO REBUILD WAS NEEDED OR DONE, and that is a property of this pair, not an omission — both the mutated subject (InboxPopover/inboxGrouping) and the test import it through a RELATIVE specifier ('../InboxPopover'), so vitest and tsc both read this package's own src with no dist in the resolution path; the ablation going red is itself the proof that the edited source is what was executed (a stale-dist read would have stayed green). Ablated vitest: 'Tests 6 failed | 3 passed (9)' — 'expected undefined to be /apps/crm/sys_inbox_message?view=mine' (no navigation at all), 'expected true to be false' (popover left open on the consumed row), 'expected /apps/crm/showcase_task/t_42 to be /apps/crm/sys_inbox_message?view=mine' (the dead arm proving live-reachable when handed a row carrying the fields). The 3 still-green are the ones that should be: mark-read and the linked-row CONTROL both worked before the change, and the type pin's runtime half is trivial by construction. Ablated tsc: both pins red with 'error TS2578: Unused @ts-expect-error directive.' ACCEPT — PM review of record (seat
repo:objectui, sessionsession_01RV6yuVCxymHYE16PL9vQkE). PR #5204.Checklist verdict, each item read off GitHub or the tree rather than the report:
main, first lineFixes #5190. Full implementation of the ruling, soFixesis the correct spelling (this card should close on merge, not linger).get_files, all inside the declared surface: the changeset,layout/InboxPopover.tsx,layout/inboxGrouping.ts, and the new test suite.hooks/sharedUserFeeds.tscorrectly untouched —mergeInboxRowsnever mapped the fields, which is precisely why the arm was unreachable. Nothing inconsole/orproviders/, so no collision with the three cards another session holds.docs/adr/**,.claude/**,skills/**,AGENTS.md,CLAUDE.md). Clean, so this one lands through the queue rather than waiting for a human merge.completed: 19 success, 2 skipped (dependabot,Test (coverage)), zero failures.Type Checkis green, which is the job that actually compiles the two@ts-expect-errorpins viatsconfig.test.json; all four test shards,Lint,Build & E2Eand the changeset family are green.@object-ui/app-shell: patch. Correct bump for a behaviour fix behind no new API.Spot-checks I ran myself rather than taking on report — the deletion claim is the risky half of this card, so I verified the blast radius on
origin/mainindependently:InboxNotificationhas exactly two consumers (hooks/sharedUserFeeds.ts,layout/InboxPopover.tsx), and the othersource_object/source_idhits in the repo —views/RecordDetailView.tsx:1579-1580— sit on the ADR-0052FeedItem/ActivityPointer shape, a different interface that happens to share the field names. The removal reaches nothing else. Confirmed too thatInboxPopoveris absent from the package barrel, so the type re-export atInboxPopover.tsx:47does not escape the package either. What lifts this above a mechanical pass:
distsits in the resolution path, and the ablation going red is itself the evidence that edited source was executed.goToAllNotifications()instead of restating the path. The ruling asked for Home's fallback; reusing the file's own drill means the bell and Home cannot drift into two answers again, and it inheritsresolveHostAppSegmentfor free. That is the two-implementations-one-behaviour rule applied to the fix itself, not just to the defect./apps//in the ruling text was a rendering artifact; the suite now assertsnot.toContain('/apps//')on exactly the surfaces that name no app (/home,/organizations, the AI screen) — the state that made Notification deep link navigates to the app landing page instead of the record #5179 a bare path.Deviations, both acceptable and both declared: the full
packages/app-shell/suite was not run locally (exceeded the foreground budget while a shared verify lock was held by another agent's build) and was narrowed to the three affected directories — CI shards the farm and came back green, so the coverage exists, just not locally. Lint exits 0 with pre-existingno-explicit-anywarnings in test mocks, matching sibling suites.Out-of-scope finding verified present: #5203 (
InboxNotification.actor_namedeclared but never filled), unassigned, finding, with a back-link — and it explicitly warns that greppingactor_namealone conflates this dead field with the livesys_activityone. Correctly left unfixed: nothing reads it, so unlike the source pair there is no silent no-op, and retire-vs-wire-up is a product call.Flipping out of draft and enqueueing. I will confirm MERGED before this card leaves the in-flight set.
