Codex extension repeatedly ask for permission #15322

Description

@Weijiang-Xiong

What version of the IDE extension are you using?

26.318.11754

What subscription do you have?

chatgpt pro

Which IDE are you using?

VS Code

What platform is your computer?

Linux, Ubuntu 22.04

What issue are you seeing?

Codex reports a broken sandbox and repeatedly ask permissions even if the edits happen locally in the workspace. "Don't ask again" simply makes no difference.

I use codex intensively everyday, and this issue pops up in 1 or 2 days. I always use default permission, GPT5.4 with extra high reasoning efforts.

What steps can reproduce the bug?

ask codex to edit any file or create a new file in the workspace.

    reverting the extension to 26.313.41514 seems to solve the problem ... people from similar threads says reverting the codex cli to an older version also helps.
