    @Andre-4711

    I can reproduce this on macOS as well, after a clean Codex restart and reinstalling the bundled browser plugins.

    Environment:

    • Codex Doctor: v0.142.0-alpha.6 · macos-x86_64
    • macOS: 26.5.0
    • Bundled browser plugin versions: browser@openai-bundled 26.616.41845, chrome@openai-bundled 26.616.41845
    • Chrome extension: installed and enabled in Chrome Default profile
    • Chrome Native Messaging Host: present and valid for com.openai.codexextension
    • macOS permissions: Automation and Accessibility enabled for Codex / Brave / System Events where applicable

    Observed behavior:

    • mcp__node_repl.js_reset succeeds with js kernel reset
    • Any mcp__node_repl.js call fails before browser setup, even a minimal call:
    nodeRepl.write('ok');

    Error:

    tool call error: tool call failed for `node_repl/js`

    Caused by:
        Mcp error: -32602: js: codex/sandbox-state-meta: missing field `sandboxPolicy`

    Steps already tried:

    • Restarted Codex Desktop
    • Removed and reinstalled chrome@openai-bundled
    • Removed and reinstalled browser@openai-bundled
    • Restarted Codex Desktop again after reinstall
    • Verified full codex doctor with escalated/network access: network, WebSocket, auth, config, state DBs, and install are OK; only TERM=dumb is reported as a terminal-display issue

    Impact:

    • Browser/Chrome extension tooling cannot initialize because node_repl/js fails before it can contact Chrome, Brave, or the in-app browser.
    • This does not appear to be caused by Chrome/Brave extension setup or macOS privacy permissions.
