What issue are you seeing?

I am seeing repeated Selected model is at capacity / server_overloaded errors that appear to be account-specific.

On the same Windows PC, same network, same Codex CLI installation, same model, same command, and nearly the same time window:

  • Free account: 11/11 requests succeeded
  • Plus account: 0/11 requests succeeded

The Plus account still had substantial usage remaining:

  • 5h limit: 93% left
  • Weekly limit: 59% left

The exact error returned by the Plus account is:

{"type":"thread.started","thread_id":"..."}
{"type":"turn.started"}
{"type":"error","message":"Selected model is at capacity. Please try a different model."}
{"type":"turn.failed","error":{"message":"Selected model is at capacity. Please try a different model."}}

Historical rollout logs for the affected Plus account also contain repeated structured errors such as:

"codex_error_info":"server_overloaded"

I found 267 deduplicated historical records containing server_overloaded.

This does not appear to be normal usage-limit exhaustion or general model unavailability, because the Free account succeeds consistently under the same local conditions.

What steps can reproduce the bug?

Environment:

  • OS: Windows
  • Codex CLI: 0.154.0
  • Model: gpt-5.6-luna
  • Same PC
  • Same Windows installation
  • Same network/public IP
  • Same Codex CLI installation
  • Same command
  • Same prompt

Reproduction command:

codex exec `
  --ignore-user-config `
  --ephemeral `
  --skip-git-repo-check `
  -C $env:TEMP `
  -s read-only `
  -m gpt-5.6-luna `
  -c 'model_reasoning_effort="low"' `
  --json `
  'Reply with exactly OK. Do not use tools.'

Steps:

  1. Log in to Account A (Free).
  2. Run the command above repeatedly.
  3. Result: 11/11 requests succeeded with OK.
  4. Log out and log in to Account B (Plus) on the same machine.
  5. Run the exact same command.
  6. Result: 0/11 requests succeeded; all returned Selected model is at capacity. Please try a different model.

Example successful Free-account response:

{"type":"thread.started","thread_id":"01a0ad75-6e3e-7d63-9281-0a39098c29dd"}
{"type":"turn.started"}
{"type":"item.completed","item":{"id":"item_0","type":"agent_message","text":"OK"}}
{"type":"turn.completed","usage":{"input_tokens":13153,"cached_input_tokens":12032,"cache_write_input_tokens":0,"output_tokens":5,"reasoning_output_tokens":0}}

Example failing Plus-account thread ID:

01a0ad75-db0f-7ca1-a914-c23db9a1155a

Result:

{"type":"turn.started"}
{"type":"error","message":"Selected model is at capacity. Please try a different model."}
{"type":"turn.failed","error":{"message":"Selected model is at capacity. Please try a different model."}}

I also tested gpt-6-astra on the Plus account.

One request succeeded, but a subsequent request returned the same capacity error.

Therefore, the Plus account is not completely unable to perform inference, but it appears to experience abnormal or intermittent serving/admission failures.

What is the expected behavior?

The Plus account should be able to invoke Codex models normally while substantial usage limits remain.

In particular, when the same machine, same network, same Codex CLI version, same model, same prompt, and same command succeed consistently with another account, I would expect the Plus account to behave similarly rather than consistently returning:

Selected model is at capacity. Please try a different model.

If this were only a temporary global capacity issue, I would also expect both accounts to be affected more similarly during the same test window.

Additional information

Additional observations:

  • Codex CLI version: 0.154.0
  • Affected account plan: Plus
  • Plus account usage at the time of testing:
    • 5h limit: 93% left
    • Weekly limit: 59% left
  • Free account:
    • Monthly limit: 100% left
  • Plus account historical rollout logs contain 267 deduplicated records with:
"codex_error_info":"server_overloaded"

Occasionally both accounts also print:

ERROR codex_models_manager::manager: failed to refresh available models: timeout waiting for child process to exit

However, this appears unrelated to the capacity failure.

The Free account produced this model-manager timeout and still completed the inference successfully with OK.

The Plus account can produce the same model-manager timeout followed by the capacity error.

This suggests that the model catalog refresh timeout is a separate issue rather than the root cause.

The controlled A/B test result is:

Free account:  11 / 11 successful
Plus account:   0 / 11 successful

The failure therefore appears to follow the account rather than:

  • the Windows installation
  • the machine
  • the network
  • the workspace
  • local Codex configuration
  • the prompt
  • the CLI installation
  • general availability of gpt-5.6-luna
  • normal usage-limit exhaustion

Could you please investigate whether the affected Plus account has an abnormal account-specific:

  • serving pool assignment
  • routing state
  • admission-control state
  • provisioning or entitlement state
  • throttling or protection state

I can provide additional sanitized rollout excerpts, exact timestamps, thread IDs, and codex doctor --json output if needed.

PII such as account email addresses, OAuth URLs, and authentication details has been intentionally omitted.

    Affected Plus account — 11 consecutive failed requests

    All of the following gpt-5.6-luna requests returned:

    Selected model is at capacity. Please try a different model.

    Near-simultaneous control comparison

    On the affected Plus account:

    2026-09-17T03:41:50.238825Z

    the request hit:

    ERROR codex_models_manager::manager: failed to refresh available models: timeout waiting for child process to exit

    and then failed with:

    Selected model is at capacity. Please try a different model.

    About 22 seconds later, on the Free account:

    2026-09-17T03:42:12.309090Z

    the same model-manager timeout occurred, but inference continued successfully:

    {"type":"item.completed","item":{"id":"item_0","type":"agent_message","text":"OK"}}
    {"type":"turn.completed","usage":{"input_tokens":13153,"cached_input_tokens":12032,"cache_write_input_tokens":0,"output_tokens":5,"reasoning_output_tokens":0}}

    Free control thread:

    01a0ad75-17c7-7443-a4c4-dc8b145b507c

    The Free account produced the model-manager timeout again at:

    2026-09-17T03:42:57.770486Z

    and again completed successfully.

    Free control thread:

    01a0ad75-c964-7061-874a-d925ab0cb2fb

    This may help correlate the affected account's requests with backend serving/routing/admission logs.

    The model-manager timeout appears to be independent of the capacity failure, since the same timeout occurred on the control account without preventing inference.

    I suspect this is because your two sessions were routed to different regions (data centers).
