It is a different version. The rest is equal.
As usually issues are from a version, I would consider it a different issue since it is a different version. But you tell me.

I am seeing the same setup-refresh failure in the Codex desktop app on Windows.
Codex desktop version:
26.924.20706, released September 25, 2026Error returned:

The setup helper completes its read-ACL phase successfully, then runtime read/execute validation fails when

This looks consistent with the long-path / longPathAware regression described in this issue. The same dependency path/hash pattern is reproduced after reinstalling workspace dependencies, so it does not appear to be a one-off corrupted download.

Additional reproduction from the Codex Windows desktop app, affecting cloud-dispatched tasks executed on the local PC.
Environment:
Even a PowerShell Get-Location command fails during sandbox setup, before the command executes. Repeated failures were recorded on 2026-09-30 between 16:35 and 16:48 (UTC+8), and again at 17:16 after a system reboot at 17:09.

These observations support a long-path compatibility problem in the setup helper, although the sandbox log does not include the underlying Win32 error code. Please consider both declaring longPathAware and handling extended-length paths explicitly in runtime read/execute validation. Enabling the OS long-path setting and rebooting alone did not resolve this reproduction.
