Environment

  • App: OpenAI.Codex 26.924.2738.0
  • ChatGPT.exe: 154.0.8037.57
  • Windows 11 Pro 25H2, build 26200.9457, x64
  • Subscription: Plus
  • Web version works normally

Issue

The Windows desktop app repeatedly hangs during startup/loading. It can remain stuck on the splash/loading state or while loading conversations and never becomes interactive.

A full clean reinstall temporarily helped: the first launch worked and I could send a test message, but a subsequent launch hung again.

Session correlation

  • Parent session: 01a0dcd7-7edc-7b02-b7ed-0a722bd8971d
  • Guardian session: 01a0dd8f-aa4a-7f33-b76f-fda8fb3fe7e5
  • Parent transcript activity ends around 09:39:51Z.
  • The next event tied to the parent appears at 11:52:55Z.
  • The Windows hang at 10:58:47Z falls inside that gap.
  • Immediately after the gap, a short guardian_review / codex-auto-review session appears. I am including this only as a correlation point, not claiming causality.
  • No approval prompt was visible in the app while it was hung.

Troubleshooting

  • Repair, Reset, reinstall
  • clean Chromium/web profile
  • clean Codex config
  • full clean install
  • codex doctor: 23 OK, 1 warning, 0 failures
  • WebSocket connectivity succeeded with HTTP 101
  • WinHTTP: Direct access (no proxy server)
  • Windows user proxy disabled; no PAC/AutoConfigURL; no proxy environment variables
  • chatgpt.com TLS issuer: Google Trust Services
  • archived_sessions directory not present
  • .sandbox/sandbox.log not present

Expected

The app should finish startup, load conversations and become interactive consistently.

I have a full redacted session JSONL ready to attach separately to this issue.

    This may be closely related to #48373 and possibly #48383.
    #48373 in particular describes the same pattern where a fresh .codex works once and the next launch hangs.
    I’m keeping this issue open for now because this report also includes a Windows Application Hang Event ID 1002 (HangType: Top level window is idle), ReportId, and correlated session JSONL timestamps that may help diagnose the underlying cause.

    I am experiencing a similar persistent startup/loading hang on Windows after an app update.

    Environment:

    • Windows 11 Pro 25H2, build 26200.9457
    • Architecture: x64 (AMD64)
    • Package: OpenAI.Codex 26.928.3736.0
    • ChatGPT.exe version: 154.0.8037.57

    The app remains on the startup loading screen. It briefly opened once under another Windows account, but subsequent launches there also failed.

    Resetting and reinstalling the app, testing a fresh Roaming\Codex profile, temporarily removing config.toml, disabling GPU acceleration, a clean boot, and a different network did not resolve the issue. Normal packaged-app activation also hangs. Test changes have been restored.

    Windows recorded repeated Application Hang events (Event ID 1002). No ChatGPT.exe Application Error event (Event ID 1000) was found around the relevant launch.

    Desktop log excerpts show:

    • fs/readFile requires an environmentId (error code -32602)
    • /tbo?limit=25 returning HTTP 404
    • A subsequent initialize_handshake_result ... outcome=success
    • The durable connection subsequently reaching state=connected

    These messages do not establish the cause of the UI hang.

    The relevant log time is 2026-10-01 14:38 UTC (23:38 JST). No matching session transcript was found in sessions or archived_sessions. Since the app is unusable, I cannot obtain a feedback session ID.

    The log excerpts below were collected during a direct executable launch; normal packaged-app activation was tested separately and also hangs.
