macOS: ChatGPT for Chrome requires access again after updates and fails saved-permission verification #48927

Description

@x-n2o

What version of the Codex App are you using (From “About Codex” dialog)?

26.924.22138 (build 11645; read from the installed app's metadata)

What subscription do you have?

ChatGPT Pro

What platform is your computer?

Darwin 27.2.0 arm64 arm

What issue are you seeing?

After updates, I have to grant “ChatGPT for Chrome” access again in macOS settings before Codex can use Chrome. The settings screen shows multiple “ChatGPT for Chrome” entries, and the selected entry has Google Chrome access enabled.

Even after enabling access, Codex cannot access the shared GitHub issue form. The exact error is:

Browser Use could not complete this action because a browser security check was unavailable. Reason: Browser Use could not request permission. Browser use cannot access https://github.com because saved browser permissions could not be verified. Please try again later. This failure may be temporary.

Retrying with a fresh Chrome tab produced the same error. A subsequent attempt in a separate Codex chat also reproduced it. This prevents Codex from filling in the GitHub issue form.

The repeated need to grant access after updates is my observed pattern; I have not isolated which update or component causes it. It is also not confirmed whether the repeated macOS entries and the saved-permission verification failure share a cause.

What steps can reproduce the bug?

Feedback ID: 01a0e70f-8084-71c1-9603-a2ff43abcbeb

Observed sequence:

  1. After an update, try using the ChatGPT for Chrome integration in Codex.
  2. Grant Google Chrome access again for “ChatGPT for Chrome” in macOS settings.
  3. Open the openai/codex new-issue form in Chrome and share it with Codex.
  4. Ask Codex to inspect or fill in the form.
  5. Observe the saved-browser-permissions verification error before the page can be accessed.
  6. Retry with a fresh Chrome tab, then from a separate Codex chat. The error persists.

What is the expected behavior?

Updates should preserve existing permissions. If access must be granted again, Codex should explain how to restore it and recognize the permission once enabled.

If verification fails despite access being enabled, the error should identify the failing component and provide an actionable recovery step.

Additional information

Observed on September 28, 2026. The original feedback chat contains the macOS settings screenshot showing repeated “ChatGPT for Chrome” entries and Google Chrome access enabled on the selected entry. No screenshot is attached to this issue.

A stale session was suggested as a possible cause, but this has not been confirmed.

These are related symptoms, not a confirmed common root cause.
