   All local scheduled tasks in Codex on Windows fail before any commands start.

   Full error:
   CreateProcess: Rejected("Failed to create unified exec process: helper_unknown_error: setup refresh had errors")

   Symptoms:

   • Both existing and newly created local scheduled tasks fail;
   • The failure occurs before any command starts; even read-only commands such as Get-Location cannot run;
   • The issue persists after restarting Codex Desktop;
   • The issue persists after creating a new local project and a new scheduled task;
   • Confirmed that the project folder exists;
   • Confirmed that the current Windows user owns the project folder and has FullControl;
   • Confirmed that the CodexSandboxUsers group has Modify permissions;
   • Created a Windows Junction alias for the project folder, but scheduled tasks still cannot run.

   Steps to reproduce:

   1. Create any local scheduled task in Codex Desktop on Windows;
   2. Have the task execute a simple local command, such as Get-Location;
   3. When the scheduled task runs, it immediately returns:
      helper_unknown_error: setup refresh had errors

   Expected result:
   Scheduled tasks should initialize the local environment successfully and execute commands.

   Actual result:
   The local sandbox fails during the setup refresh stage, and the command process is not created.

   Date first reproduced: 2026-09-28
