Windows Computer Use breaks across desktop update; recovery leaves enabled plugin without native tools #50876
Description
What version of the Codex App are you using?
Windows MSIX 26.930.3930.0, verified from the installed package and running desktop process. Managed app-server daemon, restart CLI and app-server report 0.160.0. Installed Computer Use skill: 26.930.31730.
What subscription do you have?
Not independently verified for this report.
What platform is your computer?
Windows 11 Pro, 10.0.26200, x64. Windows-native PowerShell project workflow.
What issue are you seeing?
An authorized Windows GUI acceptance workflow has been repeatedly interrupted by Computer Use connection failures. In the observed October 3 sequence, native window discovery and accessibility capture worked, then a desktop update replaced the helper/runtime during the interaction. The existing tool host retained the preceding runtime/pipe and native control failed. After documented retries and a supported managed-daemon restart, the October 4 resumed conversation still cannot operate native apps: the installed skill requires node_repl, but no such tool is callable, while the available cua_repl explicitly disables native APIs.
This report concerns the update/recovery sequence and lack of an actionable recovery path. The individual missing-tool and browser-only symptoms overlap existing issues below. A common root cause is not established.
Observed sequence / reproduction
- October 3: installed-skill
@oai/skywindow discovery and a native application's accessibility capture succeeded. This establishes native access at that point. - The first indexed click returned
coordinate input geometry is unavailable. A refreshed capture showed the desktop's update-installing surface instead of the intended target. The desktop updated to MSIX26.930.3930.0and the helper/runtime changed from81ea4d5168ddd0a3to45309f9050f7314bwhile the previous tool host remained active. - Activation subsequently failed with
Computer Use native pipe is unavailable/The system cannot find the file specified. (os error 2). Delayed discovery retry and JavaScript kernel reset / Initialize / discovery retry also failed. No GUI acceptance case began. - Configuration was backed up. The stale global entry was removed using
codex mcp remove node_repl; no replacement runtime path or pipe GUID was inserted. A supported managed-daemon restart was performed. This configuration removal is disclosed because the current missing-tool symptom is not a pristine-install reproduction. - October 4: saved restart stdout reports
status: restarted, with managed Codex, CLI and app-server version0.160.0; stderr is empty. Current process inventory confirms that the daemon is running and that a newly createdcua_replNode host and itsnode_repl.exechild use runtime45309f9050f7314b. The native helper and intended application are also running. A full desktop relaunch after the daemon restart was not independently established. - Both
computer-use@openai-bundledandunified-computer-use@openai-bundledare enabled in local configuration. The installed Computer Use skill requires anode_replsession importing@oai/sky. The callable tool inventory has nonode_replor tool-search entry. Availablecua_repltool instructions sayNative computer APIs are disabled. - The supported available observation
await cua.getState()returnsapps: []and only a Chrome extension browser, with no tabs. No native screenshot/input is possible through the exposed surface. No fresh Sky request could be issued, so the October 4 symptom is NOT a newly reproduced native-pipe exception or app-access denial.
These are observed steps, not a claim of a deterministic reproduction on a clean installation. No helper protocol client, alternative UI automation, pipe override, downgrade, cache deletion or speculative further configuration repair was attempted.
Expected behavior
The desktop should refresh or retire active native tool hosts when an update replaces their runtime/helper. If native control cannot resume, it should give a precise supported recovery instruction. Enabled Windows Computer Use should expose a compatible native tool surface matching its installed skill, or explicitly identify the failed capability before the agent starts a task.
Impact and investigation requested
Repeated failures across October 2-4 have blocked completion of an already-authorized GUI acceptance workflow. Shell/file access works; native interaction does not. Repeated restarts and handoffs without a reliable capability check consume work time and leave the workflow unfinished.
Please investigate update-time host invalidation/rebinding, the supported management of the legacy global node_repl entry, native versus browser-only session routing, and skill/tool compatibility. Provide a supported diagnostic that reports desktop/helper/host versions, expected callable tools, enabled surfaces and recovery status. Please distinguish missing tool exposure, app approval, capture failure and stale transport instead of presenting them as interchangeable permission problems.

Additional process and capability evidence, verified October 4, 2026, Pacific time (UTC-7):
The newer tool host is parented by the restarted daemon, while the helper belongs to the desktop that started before the daemon restart. This is process-lifetime evidence only; it does not prove a routing or trust failure. A complete desktop relaunch after the daemon restart has not been verified.
Sanitized saved restart result:
{ "status": "restarted", "backend": "pid", "pid": 37064, "managedCodexVersion": "0.160.0", "cliVersion": "0.160.0", "appServerVersion": "0.160.0" }Saved stderr length: 0 bytes. The current native skill file's SHA256 is

The hash identifies the observed file, not proof that installed components are mutually compatible. The agent's exposed nested-tool catalog has 189 entries, zero node_repl/Sky entries. Its separately exposed cua_repl tool explicitly disables native APIs. The supported inventory returned:
{ "apps": [], "browsers": [{ "name": "Chrome", "type": "extension", "tabs": [] }] }Fresh focused reproduction comments were added to #49844 (missing callable node_repl) and #49742 (browser-only surface). These reports should be investigated together for routing/compatibility, while retaining their distinct symptoms and unknown causality. The confirmed interruption is native GUI acceptance; browser false-denial issue #50153 remains a separate report, not newly reproduced here.
Please provide a supported post-update recovery path and a capability-health check that the user/agent can verify before spending another session on a GUI task.

Could the maintainers please prioritize this report and share a supported Windows recovery path? Native Computer Use remains blocked in this case; a clear capability health check would also help.
