[Windows][Dots][Voice] Authorized local-task handoffs remain unconfirmed while computer is connected #53206

Summary

During a ChatGPT dot voice call, authorized requests to an existing local Codex task repeatedly fail to receive delivery or execution confirmation.

Verified observations

• Connected Windows computer reports connected, attached, and authorized for tasks.
• The existing local task reports Computer Use and unified Computer Use installed/enabled, with tools loaded. Actual desktop control was not verified.
• A harmless test requested the local computer's current time.
• After approximately 23 seconds, the voice handoff returned:
  voice_parent_unconfirmed / UNAVAILABLE
  "The outcome is uncertain. Do not send another delegation or use another route to main."
• Earlier attempts also returned:
  "Not sent. The spoken context could not be confirmed yet."
• No task-execution receipt or local-time result was returned. The user reported no new instruction visible in the destination task.
• Incoming task updates and sending/reading companion chat text still worked.

Scope and expected behavior

Please investigate voice-to-task handoff confirmation and delivery correlation. This does not prove the local Computer Use plugin is defective. Exact client build has not been independently recorded; no shared root cause or fix is established. Text-input workaround was not tested in this incident.
