   Same issue here,

   • MacBook Pro M3,
     • running Sequoia v15.5
   • Codex Max subscription
   • tested on a variety of max/non-max and regular/mini models, with prompt "test"
   • tested on NPM package versions 0.63.0, 0.61.0, 0.64.0-alpha.2
