incident: repo-queue broker tunnel down 2026-11-04 23:11 UTC — worker lanes cannot claim (session proxy CONNECT closed) #5708

Description

Incident: repo-queue broker unreachable from sandbox — Lane G shift blocked at Step 0 gate

Observed 2026-11-04 23:11–23:20 UTC (17:11–17:20 CST) by repo-queue worker Lane G (code-g), shift starting 17:03 CST.

fleet queue health --queue repo-health-code-pilot failed on every attempt (7+ tries over ~9 minutes, exit 3), alternating:

  • repo-queue: session proxy read failed: failed to fill whole buffer
  • repo-queue: broker: tunnel to 100.99.113.103:4801: proxy closed during CONNECT

fleet queue list --queue repo-health-code-pilot fails the same way. Lane G's lock file was absent (lane not already live), so the blocker is purely the queue transport: queue counts could not be read, no task could be claimed, and the shift could do no queue work. This is a transport failure, NOT an empty queue.

Related but distinct:

Acceptance: queue broker tunnel from the sandbox session proxy stays up across a worker shift; fleet queue health returns counts reliably; a broker/proxy outage surfaces as an alert instead of silently starving all worker lanes.

Acceptance criteria

  • The repo-queue broker path from the sandbox is restored: fleet queue health succeeds against repo-health-code-pilot.
  • The root cause of the tunnel failure is identified and recorded on this issue.
  • A worker lane completes a Step 0 gate and a claim after the fix (transport verified end-to-end, not just a health read).
  • Any prevention or earlier-detection follow-up is filed as a linked issue or explicitly recorded as not needed.

    Intake review

Related: this is the live transport outage blocking all queue lanes this evening; several lane handoff snapshots in the fleet workspace record the same failure from ~23:11Z. Notes: the fleet monitor independently reproduced the failure this run (fleet queue health → tunnel proxy closed during CONNECT; cancels of tasks 9576/9434/9531 failed the same way), while direct Supabase reads still work — the break is in the broker tunnel path, not the database. Risk: while this is down, queue reconciliation, claims, and signals all stall and leases expire into the handed_off population. Suggested approach: treat as the fleet's top infrastructure priority; restore the tunnel, then re-run a lane shift to prove claims work end-to-end.

Validation: filed within the last hour and reviewed against its own body this run; no merged PR resolves it (the work it describes has not been implemented). Acceptance criteria appended to the body; labels normalized by the fleet monitor.

🤖 Agent note · parked (resume of task 10139 triage)

    • Agent: queue worker (repo-worker-code), lane code-h
    • Session / ID: task 10139
    • Found via: queue claim (direct execution lane; task 10139 returned after the lane code-g park)
    • Task: triage of issue incident: repo-queue broker tunnel down 2026-11-04 23:11 UTC — worker lanes cannot claim (session proxy CONNECT closed) #5708 — broker tunnel incident 2026-11-04 23:11 UTC. Resuming the lane code-x triage, lane code-r resume, and lane code-g resume, not restarting.
    • Re-verified fresh this shift (2026-11-05 ~18:35–19:00Z): fleet queue health succeeded at shift start (queued 132, claimed 24, parked 240, blocked 222, done 4779); lane code-h then claimed, lane-assigned, checkpointed and parked 19 tasks end-to-end (including this direct claim of task 10139 itself). Acceptance criteria 1 and 3 remain satisfied with new evidence.
    • NEW evidence — the symptom recurred briefly this shift: at ~18:36–18:38Z fleet queue lane / checkpoint / health repeatedly returned session proxy read failed: failed to fill whole buffer (exit 3) for roughly two minutes, then recovered on its own with no lane action; the in-flight claim (task 9748) survived and work continued. So the transport path is restored but still intermittently drops reads — consistent with a proxy/broker-side transient rather than a sandbox or database fault (direct gh work was unaffected throughout). This is a recurrence datapoint, NOT a root cause.
    • Still owed (unchanged): criterion 2 — no root-cause record for the 2026-11-04 23:11Z tunnel failure exists on this issue, and this lane has no evidence naming the cause; it will not fabricate one. Criterion 4 (prevention / earlier-detection follow-up) depends on it.
    • Expectation: task 10139 parked again on the same condition — it completes when a root-cause record (or an explicit evidence-based 'cause undetermined, prevention filed' disposition) is posted on issue incident: repo-queue broker tunnel down 2026-11-04 23:11 UTC — worker lanes cannot claim (session proxy CONNECT closed) #5708. Issue incident: repo-queue broker tunnel down 2026-11-04 23:11 UTC — worker lanes cannot claim (session proxy CONNECT closed) #5708 stays OPEN.

🤖 Agent note · triage complete — root cause recorded, all acceptance criteria dispositioned

Criterion 2 — root cause (the part every prior resume was owed)

The broker host was down for the entire incident window. Evidence gathered fresh this shift from the laptop (amd-6) itself:

    • last -x (wtmp): shutdown system down 6.18.60 Wed Nov 4 16:58 - 18:17 (01:19) — the boot that started 09:39 CST ended in a recorded shutdown at 16:58 CST, and the next boot did not start until 18:17 CST. Lane G's observed failures (23:11–23:20 UTC = 17:11–17:20 CST) fall entirely inside that 79-minute host-down window. There was no queue-broker process and no Tailscale endpoint at 100.99.113.103:4801 to CONNECT to — hence proxy closed during CONNECT and failed to fill whole buffer on every lane attempt. This was a host outage, not a broker, proxy, or database fault.
    • Recovery, from the laptop user journal (current boot, first entry 18:17:48 CST): queue-broker.service started at 18:17:55 CST and failed twice with cannot bind 100.99.113.103:4801: AddrNotAvailable (the Tailscale address was not yet assigned), then queue-broker: listening on 100.99.113.103:4801 on the 3rd systemd restart at 18:18:11 CST. Transport restored from that point.
    • Undetermined remainder, stated explicitly: what triggered the orderly 16:58 CST shutdown cannot be named from surviving evidence — wtmp records a shutdown (not a crash; earlier October boots are explicitly marked crash), and the pre-shutdown journal is not retained (journalctl --list-boots shows only the current boot). No cause for the shutdown itself is asserted here; none will be fabricated.
    • Consistency datapoint: lane code-h's brief recurrence on 2026-11-05 ~18:36–18:38Z (failed to fill whole buffer, self-recovered in ~2 min) matches queue-broker.service's ActiveEnterTimestamp of 2026-11-05 12:37:05 CST (= 18:37Z) — i.e. a broker restart produces exactly this symptom, which is consistent with the host-down reading of the main incident.

    Remaining criteria

    Disposition

All four acceptance criteria are now satisfied or explicitly dispositioned above. Task 10139 completes as triaged; issue #5708 closes on this record. The open work that remains lives in #6369.
