Description

What version of the Codex App are you using (From “About Codex” dialog)?

Desktop app 26.928.21956, build 12404; bundled codex-app-tools plugin 0.1.5. Exact running app-server version has not been independently verified for this report.

What subscription do you have?

Not independently verified for this report; omitted rather than inferred.

What platform is your computer?

macOS. Exact OS version and architecture are not included in the available sanitized diagnostic details.

What issue are you seeing?

On 1 October 2026, after enabling the documented codex_appserver override and restarting the desktop app, logs showed the server ready at 14:00:51 Australia/Melbourne. From 14:02:37, mcpServerStatus/list repeatedly returned -32601 unsupported app-server method; hooks/list returned the same error.

The /mcp UI displays "Unable to load MCP statuses". Settings > Plugins > MCPs shows plugin entries without an actionable restart control.

The affected executor still lacks read_thread and send_message_to_thread, although those tools worked earlier. The restart completed but did not resolve the errors or restore the missing tools. This blocks inspection and coordination of existing user-owned tasks through the supported tools.

What steps can reproduce the bug?

This is the observed diagnostic sequence, not a confirmed deterministic fresh-install reproducer:

  1. In the affected existing desktop task, enable the documented codex_appserver override and restart the app.
  2. Observe the server reaching ready in the logs.
  3. Open /mcp and observe "Unable to load MCP statuses".
  4. Observe repeated -32601 unsupported app-server method responses for mcpServerStatus/list and hooks/list.
  5. Inspect Settings > Plugins > MCPs: plugin entries are shown without an actionable restart control.
  6. Inspect the affected executor's available tools: read_thread and send_message_to_thread remain unavailable.

No feedback session ID is available yet. Private task identifiers and conversation content are omitted.

What is the expected behavior?

MCP status retrieval should work, and supported task read/send tools should be available or have an explicit, actionable explanation for their absence. Please investigate client/backend compatibility and tool attachment, and provide a supported recovery path that preserves existing tasks.

Additional information

The relationship between the MCP-status errors and missing task tools is unconfirmed. Client/backend compatibility and tool attachment are investigation targets, not established root causes. The exact running app-server version is not being inferred from the desktop build.

Related reports: #44044, #47510, #33947 and #44170 overlap with the missing-tool symptoms, but I have not established an exact duplicate for the paired mcpServerStatus/list and hooks/list -32601 failures. Please consolidate if maintainers determine these are the same defect.

No account email, private task identifiers, private paths, project content, credentials, raw logs, or conversation payloads are attached.

Update, 2 October 2026 (UTC): Installed the official desktop update at approximately 02:38, moving from 26.928.21956 (build 12404) to 26.928.40906 (build 12694). Fresh-session logs show mcpServerStatus/list succeeding without error at 02:38:40–02:38:42.

The existing affected task's current callable-tool catalogue still lacks read_thread and send_message_to_thread, so no owner-thread read was attempted. This is partial recovery only. The observations do not establish causation or that all reported failures are fixed; the issue remains open.
